Before You Begin
Make sure Raley IntakeForms is installed in your Jira instance from the Atlassian Marketplace.Step 1 — Watch the Introduction Video
The quickest way to understand IntakeForms is the introductory video walkthrough. It covers how to create your first form from start to finish.Step 2 — Create Your Form in Jira
In your Jira project:- Go to the IntakeForms configuration (found in project settings or the Apps menu)
- Click Create new form
- Choose the Target Project and Issue Type for submitted tickets
- Select the Jira fields you want external users to fill in
- Customize field labels, help text, and any conditional rules
- Preview the form in real time
- Save and copy the integration code
Step 3 — Publish Your Form
You have two options for making the form available: Embed on your website or intranet Paste the generated integration code into your web page’s HTML. Add to a Confluence page Install the free Raley IntakeForms Viewer plugin (available for both Confluence Server and Cloud), then add the macro to any page.Step 4 — Test It
Submit a test entry through your form and confirm the Jira issue is created correctly with all fields populated.What’s Next?
