Before You Begin

Make sure Raley IntakeForms is installed in your Jira instance from the Atlassian Marketplace.

Step 1 — Watch the Introduction Video

The quickest way to understand IntakeForms is the introductory video walkthrough. It covers how to create your first form from start to finish.

Step 2 — Create Your Form in Jira

In your Jira project:
  1. Go to the IntakeForms configuration (found in project settings or the Apps menu)
  2. Click Create new form
  3. Choose the Target Project and Issue Type for submitted tickets
  4. Select the Jira fields you want external users to fill in
  5. Customize field labels, help text, and any conditional rules
  6. Preview the form in real time
  7. Save and copy the integration code

Step 3 — Publish Your Form

You have two options for making the form available: Embed on your website or intranet Paste the generated integration code into your web page’s HTML. Add to a Confluence page Install the free Raley IntakeForms Viewer plugin (available for both Confluence Server and Cloud), then add the macro to any page.

Step 4 — Test It

Submit a test entry through your form and confirm the Jira issue is created correctly with all fields populated.
